Universal Health Services IOP Adult Therapist - PRN in SAVANNAH, Georgia

Responsibilities

Coastal Behavioral Health is currently seeking a Per Diem (PRN) Therapist for our Audlt IOP Facility:

Coastal Harbor Health System is comprised of a 114-bed hospital for children and adolescents, providing both short- and long-term inpatient care, and a 50 bed inpatient hospital serving men and women, ages 18 and up. We offer compassionate patient care to those who need it the most, serving not only local patients, but also those located across the country. Our hospitals are conveniently located in beautiful, historic Savannah, GA in a quiet setting that provides unparalleled anonymity and privacy, enabling patients to concentrate on treatment.

Responsibilities include, but are not limited to, the following tasks:

  • Perform psychosocial assessments and compile all pertinent documents to facilitate treatment planning on assigned patients.

  • Participate in treatment team sessions with completion of all treatment plan updates.

  • Monitor the progress of all patients assigned and update treatment plan, as necessary.

  • Conduct group and/or individual sessions as assigned.

  • Perform group/individual therapy and document progress notes.

  • Respond to any emergency situation involving patients in treatment.

  • Participate as a team member by cooperating with staff and demonstrate compromise and expression of opinions as appropriate.

  • Utilize chain of command in all aspects of program and follows organizations policies and procedures.

  • Contact and schedule counseling sessions with family, significant others, and involved agencies as appropriate. Ensure that there is family contact within 72 hours.

  • Attend staff meetings and in-services as required and perform duties as a positive role model for clients and colleagues.

  • Accurately, and efficiently, assess patients' aftercare needs in collaboration with the patient, physician, and treatment team.

  • Provide patients with reasonable aftercare options and discuss each option in full with the patient individually, and/or with family as requested by the patient.

  • Collect releases of information to accurately research and refer to services.

  • Initiate contact with referral sources, community mental health providers, physicians, nursing homes, residential treatment facilities, halfwayhouses, and other placement options to facilitate continuity of care.

  • Schedules follow-up appointments with a psychiatrist and therapist within a reasonable time following discharge (Psychiatrist: 30 days and Therapist: 7 days)

    Qualifications

    Job Requirements:

    Master’s level in social work or counseling with at least two years of related experience. Must be licensed or eligible for licensure- LMSW, LPC, LMFT. Must have 1+ years related experience. Experience in an inpatient setting preferred.
